Chao evolution

I'm currently working on a chao FAQ, and need some help.

I need to know how to get certain chaos.

Tails chao, i know its possible, but not sure how.

Amy chao, only in dx/adventure dreamcast, right?

Knuckles chao, the N/P/P chao looks sorta like him, but i've heard there is a way by reincarnating it six times or something like that, anyone know the full way to obtain it?
