Is there a chance that you can possibly remove or change the medal that your Chao is wearing?

The medal your Chao is wearing is based on the most recent race it won. There isn't any way to make the Chao not wear a medal anymore, but you can change it.

What if my Chao evolves or reincarnates?

After evolution, they definitely keep the medal. I'm not as sure about after reincarnation, but I know that Chao keep their headgear, animal parts, and alignment after reincarnation, so they probably keep their medal.

Yes, chao keep their medals forever after they're received. But it can be changed. They wear the badge from the latest race they've won. Sadly, there's no way to remove them. But they do look really pretty
